Supported Living for Adults – Program Overview

In March 2009, our Rochester location underwent a complete renovation, emerging as a permanent home for eight individuals with complex medical needs. We prioritize the holistic well-being of the individuals we support by addressing their medical, social, and emotional needs through a comprehensive and person-centered approach. Our dedicated staff members are deeply committed to person-directed planning, community inclusion, and customizing care routines to ensure a healthy quality of life for each person. In 2010, we expanded our program to the Leitrim home, where three individuals now reside full-time, furthering our mission to provide supportive and inclusive living environments.

Person-Centered Approach:

Our program is grounded in a person-centered approach, placing the individual needs and aspirations of each person at the forefront. Through bi-annual Person-Centered Planning (PATH) sessions, individuals collaboratively identify and pursue their dreams, goals, and actions. Our staff members, serving as primary care counselors, play an integral role in this planning process, ensuring that residents’ aspirations are realized and their voices heard.

Compliance and Funding:

As a program funded under the Ministry of Children, Community, and Social Services, we adhere to best practice models for all policies and procedures. Our unwavering commitment to compliance ensures that we consistently meet and exceed Ministry and legislated standards, guaranteeing the highest level of quality and safety for our residents.

While we welcome discussions and tours for those interested in our supported living homes, individuals seeking subsidized spots are encouraged to contact their case worker at Service Coordination or reach out to Developmental Services Ontario. We are dedicated to providing comprehensive support and fostering community integration for all individuals, empowering them to achieve their life goals and become fully integrated members of their communities.
