Community Support Programs

Comprehensive Community Nursing Support for
Developmental Service Agencies

For the past decade, our community nursing program has been a fundamental source of support for individuals with disabilities residing in group homes. We recognize the unique challenges agencies face in delivering complex care within a congregate care setting.

Dedicated to the principles of aging in place and minimizing unnecessary transfers to long-term care facilities, our specialized nursing team is unwavering in its commitment to providing comprehensive care, advocacy, capacity building, and support to staff. We prioritize enhancing agency capacity to address the intricate care needs of individuals, ensuring personalized support tailored to each person’s distinct requirements. Moreover, we offer hands-on care when needs become advanced and require the expertise of a registered professional.

Support Models:

We are proud to offer two distinct support models tailored to meet the diverse needs of our clients:

MCSS Funded Support: Our MCSS funded support model provides training, advocacy, and guidance to transfer payment agencies in the Ottawa area. This model is designed to offer comprehensive assistance in navigating the intricacies of accessing funding and resources provided by the Ministry of Children, Community and Social Services (MCSS). Through this model, we aim to empower agencies with the knowledge and tools necessary to optimize their support services effectively.

Fee-for-Service Model: In addition to our MCSS funded support, we also offer a fee-for-service model that provides more intensive and person-centered solutions. This model is ideal for individuals and agencies seeking tailored support solutions that go beyond the scope of traditional funding. Our fee-for-service offerings are designed to provide personalized, high-quality care that addresses the unique needs and preferences of each individual we serve. With a focus on flexibility and customization, we strive to deliver exceptional support that truly enhances the lives of our clients.
